VERIFY ELIGIBILITY

COMPLETE AN APPLICATION

- Applications are available Monday through Friday from 8:30 A.M. - 4:30 P.M. for pick-up at the following locations:

PERSONAL INFORMATION THAT WILL BE NECESSARY

All applicants must provide the following information or documentation for all household members:
- Income and assets
- Birth Certificates
- Social Security cards
- Declaration of Citizenship
- Photo ID (if over 18 years old.)
- Proof of Residency

If applicable, applicants must also provide:
- Disability or Handicap verification
- Marriage certificate or proof of a stable relationship
- Divorce decree
